In order to claim Unemployment Insurance (UI) benefits, you must return a completed weekly pay authorization each calendar week (Sunday-Saturday).
The form must be completed, signed and returned by mail or by using the claim drop box located at the front of the local office on the Sunday immediately following the week ending date on the top of the pay authorization form. Under no circumstances should you submit a pay authorization form prior to the appropriate week ending date.
If you are eligible to receive benefits, the Division of Unemployment Insurance will mail your check directly to you after each pay authorization form is processed. The length of time required to process pay authorizations and issue benefit checks varies with the claim load. Every effort is made to ensure that payments are made as promptly as possible.
Note that you must report your job search efforts on the back of the weekly pay authorization form.
You must also use the form to report your reemployment when you return to work.
