The age of the EAEDC applicant or recipient must be verified by one of the following documents provided the document contains evidence of the individual's age:
· Birth certificate
· Hospital birth record
· Court records
· Notification of Birth signed by appropriate hospital official
· Social Security (RSDI) benefit records
· Immigration and Naturalization records
· Baptismal certificate
· School records
· Church records or a family bible
· Insurance policies
· Employment records
· Newspaper records and local histories
· Indian agency records
· Child welfare service records
· Voluntary social service records
· Day care center records
· Head Start Program records
· Passport
· United States Census records
· Driver's license
· Other governmental records
The applicant or recipient has the primary responsibility for obtaining age verification.
