The Department of Transitional Assistance requires that the following are documented and verified at the time of the household’s initial certification for food stamps:
· Gross countable monthly income of the household: bring last 4 consecutive pay stubs (or other proof of monthly earnings)
· Citizenship status of household members (For more information see: 106 CMR 362.200)
· Monthly utility expenses of the household and use of the Standard Utility Allowance: bring utility bill receipts
· Allowable household medical expenses: bring medical bills in excess of $35 per month (only for persons age 60 or over or disabled)
· Countable assets of the household
· Household social security numbers
· Identity of the applicant: bring identification showing name and address
· Verification of disability of household members
· Actual child support payments and legal obligations to pay child support: bring receipts for dependent care costs
· Certain other financial and non-financial eligibility standards: bring bankbook or current bank statement, copies of award letters or benefits statements for Social Security pension, Supplemental Security Income (SSI), or other unearned income
