SoonerCare Plus - Application Procedures - Oklahoma

New applicants for Medicaid, as part of the eligibility process, are given information about the SoonerCare Plus program, including details about the services and plans available, where to receive services, participant rights and responsibilities, and an enrollment form.

Applicants for SoonerCare Plus who reside in the service areas covered must choose a Health Plan and Primary Care Provider (PCP) or they will be assigned to one. Parents or guardians choose on behalf of minors in the household. All household members must enroll in the same Health Plan, but each person may select a different PCP within the Health Plan. SoonerCare Plus members who are blind or disabled can choose a Health Plan different from the other family members.

Applicants can enroll through one of three different methods:

·             They can enroll at the time of their application at the Department of Human Services office (see http://www.okdhs.org/okdhslocal/ for office locations).

·             They can mail the postage-paid enrollment card enclosed in their enrollment packet.

·             They can call the toll-free SoonerCare Helpline at 1-800-987-7767 and enroll by phone. For people with hearing impairments only, call 1-800-757-5979 (TDD/TTY).

Until Health Plan enrollment is effective, Medicaid-eligible individuals receive services on a fee-for-service basis. Prior to the effective date in the program, each individual will receive a blue, plastic ID card that clearly indicates the name of their PCP and other relevant information, such as customer service numbers.
