SoonerCare Plus is a health insurance program for qualified individuals who live in Oklahoma City, Tulsa Lawton and the counties that surround these urban areas.
SoonerCare Choice is a health insurance program for qualified individuals who reside in designated rural counties.
Areas covered by SoonerCare Plus are shown darkened in the map above. They include the following:
· Counties in the Metropolitan OKC Catchment Area:
Canadian, Cleveland, Logan, Oklahoma, Pottawatomie, Lincoln, Grady, and McClain.
· Counties in the Metropolitan Tulsa Catchment Area:
Creek, Rogers, Tulsa, Wagoner, and Osage (74063, 74127, and 74126 zip code areas only).
· Counties in the Southwestern Catchment Area:
Comanche, Jackson, Kiowa, and Tillman.
Counties covered by SoonerCare Choice are shown darkened in the map above. They include the following:
