Individualized Educational Plans (IEPs) must be developed for all Students who receive Special Education services in the public schools.
When students are age 14 (or younger, if appropriate), the teams that develop the students' IEP begin discussing how students will prepare for and transition into adult life. The IEPs must include statements regarding transition service needs that focus on the course of study.
Beginning at age 16 (or younger, if appropriate), the IEPs must include statements about the transition to adult services, if appropriate. The IEP team should include representatives of participating agencies, if possible. Participating agencies may include the Department of Rehabilitative Services, the Community Services Board, the Department of Social Services; the Health Department; and the Employment Commission.
If a participating agency does not provide the transition services needed, IEP teams meet to identify other ways to achieve the transition goals.
Students must be invited to participate in IEP meetings in which transitioning issues are discussed.
