The "Health Plan" refers to any organization that is licensed as an Health Maintenance Organization (HMO) by the Oklahoma State Department of Health and contracts with the State to provide services to SoonerCare Plus enrollees.
Once a Health Plan is selected, recipients may make changes to their plan by calling the SoonerCare Help Line at 1-800-987-7767 within the first thirty (30) days of their enrollment in the program. For people with hearing impairments only, call 1-800-757-5979 (TDD/TTY).
After the first thirty days, the recipient will be "locked-in" to the Health Plan until notified of the next open enrollment period, unless they move to an area where their particular Health Plan is not offered. To change Primary Care Providers within their HMO, recipients must contact that HMO.
