Health care programs in Oklahoma are administered by the Oklahoma Health Care Authority (OHCA). A variety of different programs, each with differing funding mechanisms, eligibility standards, and cost structures, are available to meet the needs of various populations of individuals in the state.
To be eligible for medical benefits, specific financial criteria must be met. Families with children under 18 and pregnant women may also be eligible for SoonerCare, the medical benefits managed care program. Local county Department of Human Services (DHS) offices accept applications and make determinations about eligibility. (See http://www.okdhs.org/okdhslocal/ for office locations by county.)
The primary medical benefits programs are:
· Medicaid
· SoonerCare Plus
NOTE: SoonerCare Plus was Discontinued effective January 1, 2004.
· State Children's Health Insurance Program (SCHIP)
The Oklahoma Medical Assistance Application form is available for download from the Department of Human Services website. You may also download the SoonerCare Health Benefits Application Form (SC-1) in PDF format (requires Adobe Acrobat Reader). Get the form at:
