GA SOURCE Demonstration Project

What is SOURCE?

SOURCE (Service Options Using Resources in a Community Environment) provides long-term health services in a person's home or community to improve the health outcomes of persons with chronic conditions, through the development of a cost-effective, comprehensive managed care model. SOURCE is a case management model that provides the framework to manage the care of a consumer across all lines of services, diagnosis or disability.

Who Receives SOURCE services?

Individuals age 65 and older, or under 65 and disabled who are eligible for SSI and Medicaid coverage. In SFY 2002, 1,608 Medicaid consumers participated in the SOURCE program at a total cost of $7.7 million, or $4,795 per recipient.

Services available to people enrolled in SOURCE include:

·   Home-delivered meals

·   Caregiver respite services

·   Skilled services

·   Emergency response system

·   Personal Care homes

·   Personal support services

·   Adult day health services

·   Home health services

·   24-hour medical access

Levels of Care (LOC)

A key function of the SOURCE assessment process is to determine how much care a person needs. Participants are assigned one of four levels of care based on their needs for medical monitoring and assistance with functional tasks such as bathing and dressing. Participant care plans are tailored to individual circumstances.

SOURCE Levels One and Two:

These are people with substantial cognitive or physical impairments who meet the criteria for admission to a Georgia nursing facility. Level one and two participants have limited ability to live independently, and are at risk for institutionalization.

SOURCE Levels Three and Four:

These people demographically and clinically resemble the other two levels but have greater ability to function independently. Participants have at least one chronic medical condition, but do not meet Georgia nursing facility criteria.

By addressing medical and social risk factors, SOURCE works to maintain functional capacity and prevent the progression of disease.

SOURCE seeks to show that:

·   The integration of primary medical care, personal care and support services, managed by one case manager, results in fewer hospitalizations and fewer visits to emergency rooms.

·   Performance standards for service providers that exceed basic licensing requirements may contribute to improved results.

·   Levels of care assessments can predict the type, intensity, and cost of supportive services.
