DE Medicaid - HCB Waivers

Home and Community Based Waiver Services

These services provide case management to individuals who are determined to be eligible for supportive home services as an alternative to institutionalization.

There are four such Home and Community Based Waiver programs in Delaware:

The Elderly and Disabled Home and Community Based Waiver Program:

This home and community based waiver program, which is available statewide, is administered by the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD).

Services offered include all regularly covered Medicaid services (such as hospital, physician, lab, prescriptions) plus the following special waiver services:

·      Personal Care

·      Adult Day Services

·      Emergency Response System

·      Institutional or In-home Respite Care

Eligibility: Individuals aged 18 and over who are financially eligible and medically eligible for the Medicaid Nursing Facility program.

The Assisted Living Waiver Program:

This home and community based waiver program is administered by the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD). Services offered include all regularly covered Medicaid services (such as hospital, physician, lab, prescriptions) plus the services provided in a DSAAPD-approved and appropriately licensed Delaware Assisted Living Facility.

Eligibility: Individuals must meet the financial eligibility and medical criteria for DSS Long Term Care Institutionalized Services and program criteria determined by DSAAPD.

The AIDS/HIV Home and Community Based Waiver Program:

This home and community based waiver program is administered by the Division of Social Services. Services offered include all regularly covered Medicaid services (such as hospital, physician, lab, prescriptions) plus the following special waiver services:

·      Case Management

·      Personal Care services

·      Respite Care

·      Supplemental Nutrition

Eligibility: Diagnosis of AIDS or HIV with at least two or more chronic related medical conditions that would contribute to increased hospitalizations. Financial eligibility for DSS Long Term Care.

The Mental Retardation/Developmental Disability Home and Community Based Waiver Program:

This home and community based waiver program is administered by the Division of Developmental Disabilities Services (DDDS). Services offered include all regularly covered Medicaid services (hospital, physician, lab, prescriptions) plus the following special waiver services:

·      Case Management

·      Day Habilitation

·      Institutional or In-home Respite Care

·      Residential Habilitation

Eligibility: Only individuals who are being served by DDDS are eligible. If they are not already Medicaid eligible as SSI recipients, DDDS determines eligibility by using financial criteria applied to those institutionalized and receiving Medicaid.
