Applications for the ABC Special Needs Voucher must be in the name of the parent(s), legal guardian(s), or foster care parent(s). If the parent needing services is under 18 years of age, the application must be made in the minor parent’s name.
All pages of the original application must be returned with original signatures. Applications are checked to see if they are complete and include all supporting documentation. If complete, it will be reviewed for approval or denial within five (5) working days.
On the ABC application form, the applicant must include all people living in the household, their age, and relation to the applicant. If a child lives with married parents/guardian/foster parents, both adults must be working, disabled, or attending an educational or training program in order to be eligible for assistance. Adults who live together but are not married are considered part of the family group if they are parents of the same child. All work/school or training program information must be included on both parents. Stepparents and minor stepchildren residing in the home are also included in the total family size and must meet all work/school or training program requirements.
An applicant may be asked to submit a copy of their most recent tax return and child(ren’s) birth certificates to verify eligibility.
Parents and guardians who are permanently disabled must attach a copy of their most recent SSI letter to the application.
Parents or guardians who get married, or were separated at the time of application but reunite, must notify ABC Special Needs about their change of marital status within ten (10) days of the event.
Applicants who are in the process of obtaining a divorce must attach a copy of their petition for separate board and maintenance or a notarized statement lisiting spouse’s name, address, date of separation, and any type of support (cash, utility, or rent payments) given to the family.
Provisional approvals may be available for applicants who are currently unemployed but have a job scheduled to begin within the next seven (7) days. For these applicants, an application (minus check stubs) should be submitted several days prior to their scheduled employment date.
