The goal of this program is to encourage and assist persons who currently reside in a medical institution to move to and live in community housing. The program also assists persons living in a community already to avoid going into a medical institution by accessing this program.
After using 30% of their own income to pay a portion of their rental expense, eligible persons receive monthly financial assistance from this program to be used to pay the balance of the housing’s monthly rental cost. Approved payments are based on the prevailing United States Department of Housing and Urban Development (HUD) Fair Market Rent guidelines of the applicable low-rent housing program in the area where the person’s residence is located.
The fair market rent used shall be that for:
· A one-bedroom unit, or
· A proportionate share of rental costs in living units containing more than one bedroom.
Assistance for rent is equal to the rent paid or the maximum allowable rent (if less), minus 30 percent of the gross income of the individual consumer.

An eligible person may also receive one-time assistance to acquire necessary household furnishings and supplies. This is limited to a maximum of $500.
Eligibility: In order to be considered for assistance, applicants must meet all of the following minimum criteria:
· The applicant shall be an adult recipient of the HCBS waiver program.
· The applicant shall have been discharged from a medical institution on or after July 1, 1995, and immediately before receiving HCBS services, or is at risk of institutional placement.
· The applicant shall have been determined ineligible or be on the waiting list for rent subsidy programs under the U.S. Department of Housing and Urban Development or any other available rent subsidy programs.
· The applicant shall be financially responsible for rent or housing costs.
Applications for the rent subsidy program (Form 470-3302) may be obtained at any county office of the department. You can find an application in the Iowa DHS Employees' Policy Manual, Social Services, Health-Related Services, in the Manual Appendix for Chapter 16K. (Applications can be downloaded in *.pdf format for Adobe Acrobat Readers.)
A person who wishes to apply shall complete Form 470-3302, Application for HCBS Rent Subsidy and Household Assistance, and provide verification of the following:
· The person’s estimated monthly income for the 12 months following application, including written evidence from the income sources used to determine that income.
· Written evidence from sources of local rental assistance available in the applicant’s community that the applicant has applied for that rental assistance and that the applicant has been denied or placed on a waiting list.
Applications should be submitted to the Department of Human Services, Division of Mental Health and Developmental Disabilities, Hoover State Office Building, Des Moines, Iowa 50319–0114.
