The Waivered Home Care Services program (sometimes called the "Model Waiver") is one of the HCBS waivers, and has been in existence since 1982. It was initially designed to serve children up to three years old who were ventilator-dependent. The waiver was amended several times to include oxygen-dependent children up to age 21.
Waivered Home Care Services are currently available for individuals up to age 21 who are either oxygen or ventilator-dependent. A reasonable expectation must exist that the recipient's medical and nursing needs can be met adequately in the recipient's place of residence or in a day care facility. Further, there must be a need for continued nursing supervision and monitoring.
There are two services provided under this Waiver program:
· In Home Private Duty Nursing Services
Services may be provided by an RN or LPN with a current background in pediatric critical care nursing.
· Medical Day Care Services
Services must be individualized and categorized based on the medical diagnosis, medical condition, the level of care to be administered and the required number of one-on-one hours the child will need by the nurse per day. Services provided include nursing, physical therapy, speech therapy, social work, registered dietitian and transportation services. Services are provided on a twelve (12) hours or less basis, five (5) days or less per week in accordance with the recipient's health care needs.
