The Advantage Waiver provides non-institutional long-term care services to people 65 years of age or older, or individuals age 21 or older if they are physically, but not cognitively, disabled.
Participants must be eligible for Medicaid and reside in the service area, but not in a licensed facility or institution. Applicants must complete a Medical Assistance Application Form. Applicants may also apply for a State Supplemental Payment at the time of Advantage Waiver application.
A wide variety of Home and Community Based Services are available through the waiver.
