DE ASSIST - Application for Social Services and Internet Screening Tool

Delaware ASSIST is a web site that allows individuals and community based organizations access to screen and apply for a broad range of social programs. The self-screening questionnaire can tell you which Social Service Programs you and the members of your household may be eligible to receive. The application lets you apply online for Social Service Programs offered by the State of Delaware.

ASSIST asks questions about members of the household in order to evaluate possible eligibility for Social Service Programs offered by the State of Delaware. You need to enter correct information about the members of the household for ASSIST to accurately screen.

At the end of the screening questionnaire, you can apply for medical assistance, food stamps, cash assistance and long term care. If you choose to apply, the information you give in the screening questionnaire will be automatically transferred to the application.

You do not have to fill out the self-screening questionnaire. You can go directly to the online application and apply for all of the services you choose.

The information you enter is confidential. If you do not to want to apply, the information will be erased at the end of the questionnaire. You can always apply for any service provided by the State of Delaware. The screening questionnaire can only tell you what you might be able to receive. You need to apply for the services in order to see if you are eligible.

About ASSIST

ASSIST is a web site that enables you to screen and apply for a broad range of social programs.

ASSIST serves as a single access point for:

·          Medical Assistance (Medicaid, Delaware Healthy Children Program, and Qualified Medicare Beneficiary)

·          Long Term Care

·          Cash Assistance (TANF and General Assistance)

·          Food Stamp Benefits

·          Child Care Services

ASSIST also provides screening for the programs mentioned. Screening allows a user to provide basic information to determine if they are potentially eligible for a social service.

Depending on the programs selected, the Application process can take approximately 20-45 minutes to complete. The screening option can be completed in as little as five minutes.

What are the benefits of ASSIST?

·          Individuals who can't get to the office during normal business hours can apply for or renew benefits online. By using ASSIST, consumers, and the professionals who help them, can apply or renew at any time during the day or night from a home, library, healthcare clinic, community center or any location with Internet access.

·          Individuals have up to 30 days to complete the Application process, and can log in and out of the system as they please.

·          Although face-to-face interviews are still required for some programs, application processing can begin the moment the Application is submitted electronically.

·          When a person hits the "Submit" button, the system seamlessly sends it to the appropriate state agency.

·          ASSIST uses the latest technology to make the program easier for people with disabilities to use, reducing any barriers for those with visual, hearing, physical or cognitive disabilities. The Application process is compliant with the Americans with Disabilities Act (ADA).

Who is eligible to apply?

Anyone with Internet access can use ASSIST, whether they are applying for services for their own family or helping someone else apply. There are eligibility requirements for specific benefits.

After submitting the application, applicants will receive a letter from the program they applied to telling them if they are eligible for services, and providing instructions on the next steps.

Secure, Private and Confidential

An encryption process protects information during electronic transmission on ASSIST. A user identification/password ensures confidentiality once the application is received. Individuals can use their unique user identification/password to suspend the application for up to 30 days in order to gather additional information.

What social service programs can be accessed through ASSIST?

Medical Assistance (Medicaid, Delaware Healthy Children Program, and Qualified Medicare Beneficiary) - Medicaid furnishes medical assistance to eligible low-income families and to eligible aged, blind and/or disabled people whose income is insufficient to meet the cost of necessary medical services. Medicaid pays for: doctor visits, hospital care, labs, prescription drugs, transportation, routine shots for children, mental health and substance abuse services. Under the Qualified Medicare Beneficiary Program (QMB), if an individual has a low income, DSS may pay his Medicare premiums and, in some cases, other "out-of-pocket" expenses such as deductibles and coinsurance. The Delaware Healthy Children Program is a low cost health insurance program for Delaware's uninsured children.

Long Term Care offers a wide range of assistance, services, or devices provided over an extended period of time and designed to meet medical, personal and social needs in a variety of settings or locations to enable a person to live as independently as possible.

Cash Assistance (TANF and General Assistance) is available to low-income people/families based on rules and standards established by the state Division of Social Services.

Food Stamp Benefits - Food Stamps is a program that enables low-income families to buy a variety of food that is the basis for better nutrition. Benefits are issued electronically to the family's Delaware Food First card each month. This is an Electronic Benefits Transfer (EBT) card. The Food Stamp recipient uses this card at local grocery stores to access his/her food stamp benefit.

Child Care Services - Child Care is a program that provides support for families with young children to enable the caretaker to hold a job, obtain training or meet special needs of the child. Child care may also be provided in child abuse cases to help protect the child. If you are working, you may have to pay a portion of your child care expenses. You may get a list of Child Care Centers and homes in your area from your DSS Case Manager. You will also be able to get more information on the program rules and fees.
