SoonerCare Choice - Enrollment Ineligibility - Oklahoma

People excluded from participation in SoonerCare Choice include, among others:

·             Individuals receiving services in a nursing facility, in an intermediate care facility for the mentally retarded (ICF-MR) or through a Home and Community Based Waiver

·             Individuals privately enrolled in a Health Plan or in a pilot project or demonstration model other than a Primary Care Provider system

·             Individuals who would be traveling more than 45 miles or an average of 45 minutes to obtain primary care services

·             Individuals living in an area excluded from the program

·             Medically needy individuals and individuals with a retroactive eligibility period only

·             Individuals in the custody of the Department of Corrections or the Department of Mental Health

·              Illegal aliens

·             Children in subsidized adoptions

·             Individuals who are dually eligible for Medicare and Medicaid
