SoonerCare Choice - Enrollment Requirements - Oklahoma

SoonerCare Choice enrollment is limited to Medicaid-eligible persons or persons categorized as Aged, Blind, or Disabled who are not eligible for both Medicaid and Medicare.

SoonerCare Choice enrollees must live in rural areas, outside the urban areas covered by the SoonerCare Plus program. Once enrolled, participants may choose a Primary Care Provider, and can make changes to the PCP up to four times each year.
