To be eligible for the Elderly/Disabled Home and Community Based Waiver (HCBW), a person must be age 18 or older, be financially eligible for Medicaid, and need the type of care typically received in a nursing home. A waiting list applies.
Services in the Elderly/Disabled Waiver include:
· Case Management
· Personal Care II
· Personal Care I (Homemaker)
· Attendant Care
· Adult Day Health Care/ADHC Nursing
· Home Delivered Meals
· Environmental Modifications
· Respite Care
· Personal Emergency Response System
· Incontinence Supplies
· Companion
