Group Adult Foster Care provides personal care in either an assisted living community or an elderly- or public-housing complex to certain individuals at imminent risk of institutionalization.
To receive Group Adult Foster Care, a member must:
· be 22 years of age or older;
· meet the financial criteria for MassHealth in the community;
· have a physician's authorization confirming the individual's risk of institutionalization;
· need daily assistance with at least one activity of daily living, such as assistance with bathing or dressing; and
· have approval from the aging service access point.
