HAWK-I (Healthy and Well Kids in Iowa) is a private insurance option that may be purchased from participating private insurance companies to cover certain medical services for children when the families income is too great to qualify for Medicaid but does not exceed 185% of the poverty guidelines.
You can call HAWK-I Customer Service at 1-800-257-8563 and ask that an application be mailed to you. You can also download an application form in PDF format (requires Adobe Acrobat Reader) from:
