Developmental Disabilities Services - HCBW ICF/MR Eligibility - Oklahoma

To be eligible for the ICF/MR Waiver, the person must:

·             Meet HCBW eligibility requirements

·             Be determined to have a disability with a diagnosis of mental retardation, by the Social Security Administration; or the OHCA Level of Care Evaluation Unit

·             Be age six or older

·             Be determined by the OHCA to meet the ICF/MR Institutional Level of Care requirements

A determination of need for ICF/MR Institutional Level of Care does not limit opportunity for the individual to participate in community services.
