SoonerCare Choice - Native Americans - Oklahoma

All Indian Health Service (IHS) eligible American Indians must select a SoonerCare Choice Primary Care Provider/Case Manager (PCP/CM) even if you get all your health care through an IHS, Tribal or Urban Indian Clinic.

Call the SoonerCare Helpline at 1-800-987-7767 or for hearing impaired only, call 1-800-757-5979 (TDD/TTY) to find out which doctors or providers are signed up with SoonerCare Choice. Your PCP/CM can give you referrals for medically necessary health care from a specialist. He or she will help you get the care you need.

American Indians enrolled in SoonerCare Choice can receive services through an IHS, Tribal or Urban Indian Clinic, even if the facilities are not signed up with the SoonerCare Choice program. You do not need a referral from your PCP/CM to get services.

Providers at these facilities can be signed up with SoonerCare Choice. You can choose a provider from these facilities to be your PCP/CM if they are with SoonerCare Choice.

If you need care, like contract health services or hospital care, that is not available from one of these facilities, you need a referral from your PCP/CM before the service can be paid for.
