SoonerCare Choice - Health Plan Changes - Oklahoma

The recipient may change providers up to four times per year under SoonerCare Choice. To change providers, the recipient must call the SoonerCare Helpline at 1-800-987-7767. For people with hearing impairments only, call 1-800-757-5979 (TDD/TTY).

The Oklahoma Health Care Authority (OCHA) may also change a member’s enrollment from one Primary Care Provider to another under certain circumstances, such as:

·             If a member has been habitually non-compliant with the documented medical directions of the provider

·             If a member poses a threat to employees or other patients of the PCP or as a result of a grievance determination by OCHA

·             In cases where reliable documentation demonstrates that the physician-patient relationship has so deteriorated that continued service would be detrimental to the member the provider or both; or

·             If the member's illness or condition would be better treated by another type of provider
