The Division of Medical Assistance implemented the ShepherdCare program at the Shepherd Spinal Center in Atlanta, a facility for people with severe disabilities.
ShepherdCare provides care through an outreach program managed by advanced practice nurses who coordinate medical care for severely disabled people.
The Division of Medical Assistance reimburses the Shepherd Center a flat monthly fee per recipient for managing each person's care. The program is available in 20 Georgia counties: Barrow, Bartow, Carroll, Cherokee, Clayton, Cobb, Coweta, DeKalb, Douglas, Fayette, Forsyth, Fulton, Gwinnett, Henry, Newton, Paulding, Pickens, Rockdale, Spalding, and Walton.
· Arranges the same types of service available to consumers under the Independent Care Waiver Program: case management, personal support services, specialized medical equipment and supplies, occupational therapy, respite care services, counseling, home modification, and personal emergency response services;
· Provides direct care and coordinates all needed medical services, equipment, supplies and physician visits;
· Maintains a 24-hour information call center for members to help determine the appropriate response in an emergency;
· Provides regular check-ups to evaluate a person's well being and health status, including the needs of the disabled person's family and other care givers.
